Please complete this form to register your firm for access to the FPM Online Services website. This online service provides access to client's policies, financial reports, product stationery and other useful tools.

During registration you will be required to provide a password. Once your registration request has been approved, we will email you a Master Account username. Your username and password can be used together to access FPM Online Services.

The FPM Funeral Director Online Services website is only available to Funeral Directors and their employees and is not intended for use by other persons. When you register for use of the FPM Funeral Director Online Services website, you are agreeing to be bound by the following terms and conditions of use and the terms and conditions otherwise stated on this website, including but not limited to the General Terms and Conditions of Use and the statements on the Privacy and Security page. Use of this website is subject to the terms outlined on this website.

Privacy
FPM collects personal information from you to verify who you are and which Funeral Director you work for, to enable the provision of services available via this website to you and to contact you when necessary. FPM may also provide your personal information to service providers used to assist FPM with its administration and Funeral Director communications. This information is provided on a strictly confidential basis and will only be used for the purposes of the Lifeplan Group. Without the mandatory information, FPM cannot make the Funeral Director Online Services website services available to you. If you no longer wish to use the website, or you wish to access your personal details, please contact us.

Security
FPM will send you a UserID. To maintain security, it is your responsibility to keep your Password and UserID confidential and store them separately at all times. You must not disclose this information to any other person. Access will be provided to you on the correct entry of a UserID and Password. You agree to notify FPM and discontinue use of your password if you discontinue employment with the Funeral Director who was your employer at the time of requesting the UserID.

Indemnity
You agree to indemnify FPM and Lifeplan and all users of the site against any claim, loss, cost, damage, action or expense, whether direct or indirect, which FPM or Lifeplan or such user may experience as a result of your use of this site, your failure to keep your Password confidential, the use of the site by any person on your behalf or the use of your Password by any other person.

Liability of FPM and Lifeplan
FPM will take all reasonable steps to ensure the information provided on this site is accurate and up to date as at the date the site is last published. However, neither FPM nor Lifeplan warrants the accuracy of the information provided, or its suitability for any particular use. Subject to any terms implied by law and which cannot be excluded, neither FPM nor Lifeplan will be liable for any loss or damage arising from the use of, or any omission, error or inaccuracy in, information provided on this website.
